Case Review & Event Trigger Tools

The "trigger tool" methodology describes a way to greatly enhance the chart or case-note review process by rapidly identifying events that may suggest the presence of an adverse event or omission of care. Metrixcare has developed a general trigger tool for case review that can be used to quickly build a profile of safety and quality across a range of known clinical issues by reviewing as few as 20 cases.

 

Including a manual case review component into your incident or adverse event reporting program can help assess the accuracy and validity of voluntary reporting initiatives while uncovering hidden issues that may not be visible through other methods of error detection.
